Hold Harmless Agreement

I hereby understand that myself and/or team is a willing a participant in any athletic activity and/or event. I understand there is an element of risk involved in activities of this nature and I agree that by signing this form that I release, indemnify, and hold harmless the Lafourche Parish Government, and Lafourche Parish Recreation District No. 1, its supervisors, organizers, and employees from any claims arising out of injury or accident. By adding to cart, checking out and sending payment to Lafourche Parish Recreation District No. 1 I am in agreement to these terms.
